About the position The RN Transition Navigator at Ochsner Health serves as a vital liaison among patients, caregivers, healthcare providers, and multidisciplinary teams. This role focuses on facilitating patient movement through the healthcare continuum, ensuring quality and cost-effective outcomes while assisting with discharge planning and alternative care options. Responsibilities • Manage various aspects of the planning process for patient movement through the healthcare system. • Utilize care management policies and procedures, applying evidence-based criteria and level of care. • Manage the care of assigned patients based on individual needs. • Prepare, execute, and reinforce post-discharge care plans. • Remain knowledgeable on current laws, accreditation standards, and regulatory agency requirements. Requirements • Registered nurse diploma required; Associate's or bachelor's degree in nursing preferred. • 2 years of related experience required. • Current RN license in state of practice required. • Basic Life Support (BLS) certification through the American Heart Association required. • Certification in Case Management (CCM) and Health Care Management (CPHM) preferred. • Proficiency in using computers, software, and web-based applications. • Effective verbal and written communication skills.
