About the position The RN Transition Navigator at Ochsner Health serves as a vital liaison among patients, caregivers, healthcare providers, and multidisciplinary teams. This role focuses on facilitating patient movement through the healthcare continuum, ensuring quality and cost-effective outcomes while assisting with discharge planning and alternative care options. Responsibilities β’ Manage various aspects of the planning process for patient movement through the healthcare system. β’ Utilize care management policies and procedures, applying evidence-based criteria and level of care. β’ Manage the care of assigned patients based on individual needs. β’ Prepare, execute, and reinforce post-discharge care plans. β’ Remain knowledgeable on current laws, accreditation standards, and regulatory agency requirements. Requirements β’ Registered nurse diploma required; Associate's or bachelor's degree in nursing preferred. β’ 2 years of related experience required. β’ Current RN license in state of practice required. β’ Basic Life Support (BLS) certification through the American Heart Association required. β’ Certification in Case Management (CCM) and Health Care Management (CPHM) preferred. β’ Proficiency in using computers, software, and web-based applications. β’ Effective verbal and written communication skills.
