OVERVIEW: A growing Home Health Agency in the San Francisco Bay Area is seeking a self-disciplined and detail-oriented Remote Home Health Quality Assurance Specialist (RN/LVN/LPN) to join our team remotely. This is a flexible contract (1099) position, ideal for experienced RNs, LVNs, or LPNs passionate about delivering high-quality care through OASIS, chart, and clinical documentation reviews as well as training clinicians in the field on best practices in home health. QUALIFICATIONS: • Graduate of an approved school of professional nursing, with current licensure in the practicing state(s). • At least three (3) years of clinical nursing experience required (Home Health preferred). • One (1) year of OASIS QA review experience required. • LVN/LPN candidates must have at least three (3) years of Quality Assurance experience in Home Health. • Proficiency in documentation, time management, and organizational skills. • Experience with Kinnser/WellSky or similar home health software is highly preferred. • OASIS certification is highly desirable. • Experience with PT Eval/Visit QA highly preferred. RESPONSIBILITIES: • Conduct comprehensive clinical record reviews for clinical notes/charting, starts of care, resumptions, recertifications, and discharges. • Ensure OASIS assessments accurately reflect patients’ current health status. • Perform patient chart audits to identify and resolve documentation discrepancies. • Collaborate with the team to develop educational programs that improve documentation quality and compliance. • Maintain proficiency in CMS and industry best practices for OASIS standards.
