As an LVN, you will provide professional nursing care to patients in their homes under the supervision of a Registered Nurse (RN). Your role is vital in helping patients maintain their health and independence in the comfort of their homes.<br><br>

<strong>Key Responsibilities:<br></strong><br>
</div><ul>
<li>Provide skilled nursing care according to the physician’s plan of care.<br><br>
</li>
<li>Administer medications and treatments as prescribed.<br><br>
</li>
<li>Monitor patient condition and report any changes to the supervising RN or physician.<br><br>
</li>
<li>Document all nursing services accurately and in a timely manner.<br><br>
</li>
<li>Educate patients and families on health maintenance and care plans.<br><br>
</li>
<li>Collaborate with interdisciplinary team members to ensure coordinated care.<br><br>

<strong>Qualifications:<br></strong><br>
</div><ul>
<li>Active LVN license in the state of CA<br><br>
</li>
<li>Current CPR/BLS certification.<br><br>
</li>
<li>Valid driver’s license and reliable transportation.<br><br>
</li>
<li>Minimum of [1 year] experience as an LVN (home health experience preferred).</li>
