Licensed Vocational Nurse (LVN) - Home Health
We are currently seeking experienced and dedicated Licensed Vocational Nurses (LVN) to join our dynamic team. As an LVN with A-List Therapy Group, you will play a crucial role in delivering compassionate and comprehensive healthcare to our patients.

Locations: (Los Angeles County, San Bernardino county, Orange County, Riverside County, Ventura County)
Responsibilities:
Β Initiate the plan of care and make necessary revisions as patient status and needs change.
Β Use health assessment data to determine clinical diagnosis and treatment plans.
Β Provide health care instructions to the patient as appropriate per assessment and plan.
Β Receive and carry out physician orders.
Β Notify the patient physician, RN, and/or Clinical Supervisor immediately of any changes in the patientΒs condition.
Β Include the patient and the family in the planning process.
Β Document direct patient services, ensuring thorough coordination and collaboration with physicians and other disciplines
Requirements:
Β Current California LVN license.
Β Driver license, Auto insurance
Β Valid CPR card
Β Ability to travel to the prospective patient's place of residence
Compensation: $40-$55
This is a 1099 position with payment on a per-visit basis. Compensation.
