As a Case Management Specialist, you will be responsible for assessing member needs, communicating with Primary Care Providers, maintaining updated care plans, and participating in Interdisciplinary team meetings.
Key Responsibilities:
- Conduct assessments and create individualized care plans to address member needs and goals
- Collaborate with the Care Team to improve health outcomes and address social determinants of health
- Communicate effectively with members, Primary Care Providers, and community-based organizations
- Maintain accurate and up-to-date records, including care plans and member interactions
- Participate in Interdisciplinary team meetings to discuss member care and outcomes
Requirements:
- 3+ years of experience in a health-related field
- Bachelor's Degree or equivalent experience
- Strong organizational and communication skills
- Ability to work independently and maintain professional relationships
- Proficient in Microsoft Office Software and computer systems
Preferred Qualifications:
- Medicare and Medicaid experience
- CRC, CDMS, CRRN, COHN, or CCM certification
- Managed Care experience
- Experience working with geriatric special needs, behavioral health, and disabled populations
- Bilingual (English/Spanish; English/Creole)
