Job Overview
We are seeking a dedicated and compassionate Care Coordinator (certified medical assistant/ registered medical assistant) to join our healthcare team. The ideal candidate will play a crucial role in managing patient care, ensuring seamless communication between patients and healthcare providers. This position requires a strong background in various medical fields, including chronic care management and remote patient monitoring to effectively support our diverse patient population.

Location: Remote (FL, GA, NJ, NY, OH, TX)

    Duties
  • Coordinate patient intake processes, ensuring accurate collection of medical history and vital signs.
  • Provide administrative support within medical settings, including managing electronic health records (EHR) systems.
  • Assist in medication administration and educate patients on their treatment plans.
  • Collaborate with healthcare professionals to facilitate patient care across various departments such as outpatient clinics, emergency medicine, and inpatient services.
  • Utilize knowledge of medical terminology, anatomy, and physiology to communicate effectively with patients and staff.
  • Support the delivery of care in specialized areas such as geriatrics, internal medicine, and medical-surgical units.
  • Participate in the management of patients in Level I and Level II trauma centers, ensuring high-quality care during critical situations.
  • Conduct follow-ups with patients post-treatment to monitor recovery and address any concerns.
  • Conducts interviews and questionnaires for eligible patients. Interacts and communicates synchronously and asynchronously in person, via telephone, video, email or text.
    Experience
  • Certified/Regiatered Medical Assistant with at least 4 years of experience
  • Must have RPM (Remote Patient Monitoring) experience
  • Must have CCM (Chronic Care Management) experience
  • A plus if they already comfortable using Microsoft Teams for communication
  • Strong patient engagement and communication skills
  • Willing to work Pacific Time Zone schedule
