Certified Patient Navigator – Florida (Part-Time, Hourly, Remote)
Location: Remote (Florida residents only)
Job Type: Part-time

We are hiring a Certified Patient Navigator to support patients through their care journey. This remote role requires an active Florida state Patient Navigator certification, hands-on navigation experience, and strong knowledge of Medicare and Medicaid.
Responsibilities
- Guide patients through appointments, referrals, and follow-up care
- Assist with Medicare and Medicaid eligibility, benefits, and coverage
- Provide clear and supportive patient education
- Maintain accurate patient records and documentation
- Communicate with clinics, care teams, and community partners
- Identify barriers to care and connect patients to needed resources
- Ensure full HIPAA compliance and patient confidentiality
- Support patients remotely via phone, email, and telehealth Required Qualifications
- Active Florida state certification as a Patient Navigator
- Minimum 1 year of direct Patient Navigator experience
- Strong knowledge of Medicare and Medicaid
- Experience with EHR systems and scheduling platforms
- Excellent communication and organizational skills
- Ability to work independently in a remote setting
- Must reside in Florida and be legally authorized to work in the U.S. Additional Information

- Hourly rate based on experience and certification
- Level II background check required
- For a Florida-certified Patient Navigator role, residency in Florida is the norm and should be required.
