About the position
- Responsibilities
- Assists with implementation of health management activities/programs for membership, coordinating all aspects of assigned programs.
- Facilitates programs to participants with a focus on high-risk members and follows/updates established procedures.
- Conducts research and analysis of reports on a monthly, quarterly, and annual basis.
- Assists with the development of strategies to increase effectiveness and participation in health management programs.
- Establishes and maintains effective communications with provider networks, group representatives, and members.
- Provides health coaching telephonically or in person for assigned health programs, utilizing data systems to document patient information appropriately.
- Assists with identifying areas needing health management programs and monitors quality indicators to develop programs for improvement.
- Develops patient/provider educational materials for members and providers.
- Requirements
- Bachelor's Degree in Nursing, Health Administration, Health Promotion, Exercise Science, Education/Training, Social Work, or a health-related field.
- 1 year clinical experience in defined specialty and/or facilitating health education and prevention programs; OR 2 years experience in Health Administration or a health-related field.
- Nice-to-haves
- Bachelor's degree in Health Education/Health Management, Healthcare Administration, Social Work, or Health Promotion.
- 3 years of healthcare program management experience.
- Active, unrestricted LBSW (Licensed Bachelor of Social Work) licensure from the United States and in the state of hire.
