Wellbox is a rapidly growing healthcare company focused on empowering people to live healthier lives through comprehensive preventative care solutions delivered by an elite team of experienced nurses. We are currently seeking tech-savvy LPN Patient Care Coordinators to conduct monthly telephonic outreach to chronically ill patients, developing personalized care plans that address their health challenges.
Essential Functions:
ย Manage patientsย healthcare needs via virtual phone conversations.
ย Document visits using technology platforms and EHRs.
ย Develop personalized care plans addressing physical, mental, and preventative health.
ย Coach patients on treatment plans, including nutrition and wellness.
ย Connect patients with resources and prepare them for medical appointments.
Requirements:
ย Active Compact LPN License.
ย Minimum two years of clinical experience; care coordination experience preferred.
ย Proficient with Electronic Medical Records and Microsoft Office.
ย Excellent communication and problem-solving skills.
ย Candidates who reside in Mountain Standard Time (MST) are preferred; candidates in Pacific or Central time zones will also be considered.
Position Type and Expected Hours of Work
We are seeking full-time LPN team members who can work 40 hours per week, between the hours of 8 am ย 6 pm MST, Monday ย Friday.
Pay Structure
ย Orientation + Training (First two months): $20 hourly
ย Post-Orientation: $22 hourly, plus bonus incentive
ย Monthly Bonuses up to $525
ย Referral Bonuses up to $1000
