<p><strong>Quality Outreach RN β Care Coordinator (Contract | Remote)</strong></p><p><br></p><p>We are seeking an experienced and motivated <strong>Quality Outreach RN</strong> to support quality outreach initiatives and member engagement efforts. This contract opportunity is ideal for nurses with strong <strong>HEDIS, quality improvement, and managed care experience</strong> who thrive in a remote, performance-driven environment.</p><p>The Quality Outreach RN will leverage clinical knowledge and quality program expertise to conduct member outreach, support care gap closure, and contribute to improved quality performance and member outcomes.</p><p><br></p><p><strong>Key Responsibilities</strong></p><p>β’ Conduct outbound member outreach to support HEDIS gap closure and preventive care compliance</p><p>β’ Coordinate care across providers and healthcare settings to support continuity of care</p><p>β’ Monitor member progress and identify barriers to treatment adherence</p><p>β’ Review medical records, claims data, and HEDIS measures to identify intervention opportunities</p><p>β’ Support quality initiatives focused on improving HEDIS, Stars, and member experience metrics</p><p>β’ Document outreach activities, barriers, and outcomes accurately and in a timely manner</p><p>β’ Collaborate with clinical and quality teams to support reporting and compliance initiatives</p><p>β’ Maintain HIPAA compliance and adhere to all regulatory requirements</p><p><br></p><p><strong>Qualifications</strong></p><p>β’ Active, unrestricted RN license (compact license preferred)</p><p>β’ Required HEDIS experience, including gap closure and quality outreach work</p><p>β’ Experience with Medicare, Medicaid, or other government programs populations strongly preferred</p><p>β’ Minimum 3β5 years of clinical experience in managed care, population health, or quality programs</p><p>β’ Strong communication, organization, and documentation skills</p><p>β’ Experience working in remote or telehealth environments preferred</p><p>β’ Proficiency with EMR systems and quality documentation platforms</p><p><br></p><p><strong>Contract Details</strong></p><p>β’ Fully remote contract role</p><p>β’ Contract duration based on project timeline</p><p>β’ Standard MondayβFriday schedule (project dependent)</p><p><br></p><p><strong>Compensation Structure:</strong></p><p>β’ <strong>$45/hr β 1099 (LLC rate)</strong></p><p>β’ W2 option may be available at an adjusted hourly rate</p><p>β’ Must be able to work as an independent contractor if selecting the 1099 option</p><p>β’ Contractors are responsible for their own taxes, benefits, and insurance</p><p>β’ <strong>No C2C agencies</strong></p>
