Key Responsibilities:
- Deliver direct patient care, including medication administration and vital sign monitoring
- Develop and implement personalized care plans tailored to each resident's needs
- Collaborate with physicians, nurses, and therapists to ensure comprehensive care
- Maintain a safe and hygienic environment, adhering to infection control protocols
- Educate residents and families on health-related topics, promoting understanding and empowerment
- Maintain accurate and timely documentation in resident medical records
Required Skills:
- Current and unencumbered LPN license in the State of South Dakota
- Excellent communication and interpersonal skills
- Empathetic and patient-centered approach to care
- Strong team player with ability to work collaboratively
Qualifications:
- Completion of an accredited LPN program
- Experience in long-term care or similar healthcare setting preferred but not mandatory
