Key Responsibilities
As a Home Health Occupational Therapist, you will collaborate with the Case Manager to provide skilled therapeutic services to patients in their homes. Your role will include:
ย Conducting evaluations and addressing patient needs to promote safe and productive living.
ย Performing home safety assessments and making necessary modifications.
ย Implementing medication management strategies.
ย Assisting with wheelchair seating and positioning.
ย Administering pain management techniques.
ย Developing falls prevention programs.
ย Managing chronic diseases and dementia.
ย Training families and caregivers.
Required Skills
To excel in this position, you will need:
ย An active and unrestricted CA Occupational Therapist License.
ย Current BLS/CPR Certification.
ย A valid CA Driverยs License.
ย Knowledge of local community resources.
ย An understanding of individual development, human behavior, and cultural influences in healthcare.
ย Knowledge of homecare regulatory guidelines (preferred).
ย Experience in home health care (highly preferred).
ย Familiarity with high-acuity patients (a plus).
ย The ability to work effectively within an interdisciplinary team (highly desired).
ย Fundamentals of case management (highly desired).
ย Self-direction, empathy, and critical thinking skills.
ย Strong interpersonal and leadership abilities.
ย Excellent organizational skills.
Qualifications
ย A degree in Occupational Therapy from an accredited institution is required.
ย Prior experience in home health care is highly preferred.
ย Experience with high-acuity patients and diverse populations is valued.
