About the position
The RN Consultant position at Liberty Healthcare involves providing technical assistance and training to Service Coordinators conducting assessments using the interRAI-HC tool. This hybrid role requires both fieldwork and remote work, focusing on quality reviews and performance improvement in Home and Community Based Services (HCBS).
Responsibilities
Β In-person and virtual training and technical assistance with Service Coordinators
,
Β Applying subject matter expertise with the interRAI-HC tool
,
Β Ensuring best practices are used in the administration of the interRAI-HC tool
,
Β Conducting quality reviews of assessments and service plans
,
Β Proficiency testing
,
Β Developing and implementing performance improvement plans and monitoring efficacy
,
Β Collaborating with supervisors, managers, trainers, and educators
,
Β Data analysis and reporting
Requirements
Β Registered Nurse (RN) license
,
Β Experience conducting functional assessments of older adults and adults living with disabilities
,
Β Experience in Home and Community Based Services (HCBS) or Long Term Services and Supports (LTSS)
,
Β Experience in case management and service coordination
,
Β Experience in leadership, management, supervision, or consulting
,
Β Experience providing technical assistance and training
