We offer user-friendly solutions with advanced care coordination and AI-driven technology to enhance patient care and reduce healthcare costs.</p><p><br></p><p><strong>Role Description</strong></p><p>This is a full-time remote role for a RN in Remote Care Management in CA, AZ, NY, NV, NM, IL, FL. The RN will be responsible for providing proactive care coordination, enhancing documentation, and conducting health risk scoring using our AI-driven platform. Duties include engaging patients, collaborating with providers, and reducing ER visits through effective care management strategies.</p><p><br></p><p><strong>Qualifications</strong></p><ul><li>Proficient in patient engagement, care coordination, and health risk assessment</li><li>Experienced in remote patient monitoring</li><li>Strong clinical documentation and assessment skills</li><li>Excellent communication and teamwork abilities</li><li>Knowledgeable in EHR systems and technology integration</li><li>Active RN license in one or more of the following states: CA, AZ, NY, NV, NM, IL, FL</li><li>Bachelor’s Degree in Nursing (BSN) or higher, with relevant certifications (e.g., CCM)</li><li>Preferred: Specialty experience in Nephrology or Cardiology, and Spanish language proficiency</li></ul><p><br></p><p><br></p>
