Program Overview
As a member of our Aetna team, you will have the opportunity to make a life-changing impact on Dual Eligible Special Needs Plan (DSNP) members. These individuals are enrolled in both Medicare and Medicaid, encountering complex health and social challenges. Your expertise in compassionate communication and collaboration will help to address their full range of health care and social determinant needs.
Position Summary
- As a Social Worker, you will play a key role in the care planning process alongside the Care Manager. Your responsibilities will include:
- Conducting telephonic and face-to-face care management activities, including home visits and community interactions.
- Focusing on quality of care, compliance, improving outcomes, and reducing costs.
- Coordinating and integrating post-acute and long-term care services to prevent hospitalizations and custodial placements.
- Performing initial and ongoing assessments of members enrolled in various care programs.
- Applying social work principles to address social determinants of health in individualized care plans.
- Engaging in discussions with providers and care management staff to monitor member progress toward goals.
- Advocating for members to ensure they receive necessary care through their benefit plans and available community resources.
- Building relationships with hospital social workers and community resources to facilitate appropriate care management.
- Identifying and reporting potential quality improvement issues.
- Directing social work interventions, including psychosocial assessments and documentation of member needs and progress.
- Required Qualifications
- Must reside in the applicable county.
- At least 2 years of experience in medical social work or case management.
- New York State License in Social Work.
- Compliance with all state requirements for job duties.
- Reliable transportation and willingness to travel up to 75-90% of the time within a 50-mile radius; mileage will be reimbursed.
- Education
- Master's Degree in Social Work.
- Certification
- NY License in Social Work.

Pay Range
The typical pay range for this role is $51,256.00 - $111,351.00. Actual salary offers may vary based on experience, education, geography, and other relevant factors.
